Amenorrhea

Amenorrhea is the absence of periods. Primary amenorrhea can occur when a female does not have any menstrual periods before age 16. Secondary amenorrhea can occur when a female has previously menstruated, but then stopped having periods (for a 3 to 6 month period or more). Often a loss of period indicates pregnancy.

Your doctor will perform a number of tests to determine what is causing the lack of periods, including a medical exam, blood tests, imaging tests, or laparoscopy or hysteroscopy (minimally invasive surgeries to view internal organs). There are many reasons why women stop having their periods. These can be hormonal reasons or physical reasons.

The type of treatment needed depends on what is causing the amenorrhea. For women who are obese, weight loss may help with treatment by allowing the return of menstrual cycles. Women with polycystic ovarian syndrome (PCOS) often struggle with obesity and weight loss of 10-15% of your body weight this may lead to the return of your menstrual cycles and therefore the ability to become pregnant without the use of medications. Women with PCOS have hormonal irregularities that lead to irregular menstrual cycles or lack of menstrual cycles. It is one of the most common disorders in women who are having irregular cycles and difficulty becoming pregnant.

Other women may have amenorrhea due to problems with the signal from the brain (pituitary or hypothalamaic problems) to the ovary. These women require injectable hormones to stimulate the ovaries to release the eggs that are there. Amenorrhea may also be caused by thyroid or pituitary disorders, which may be treated with medications specific to those disorders. Less commonly, some women have genetic problems that lead to an early loss of their eggs (Turner's Syndrome). These women require egg donation in order to become pregnant. Some women can undergo premature ovarian failure where the eggs “die” off too quickly in a woman’s reproductive life time and this can lead to amenorrhea and then early menopause. These women require the use of donor eggs in order to become pregnant.
